If the touch feels uneven, some keys are heavy, or the piano no longer responds the way it should, it needs regulation, not just tuning.
You can tune a piano perfectly and still have it feel wrong to play. Tuning is about pitch. Regulation is about touch and response. Older pianos and heavily played pianos drift out of regulation over time as the felt and wood parts wear and settle.
